We are here to provide support and services, coordinate care, and expand our offerings by partnering with community organizations. Serving all ages, from birth to aging, we want to ensure that all individuals with disabilities in our community receive support.

Every March, we work with Access Gallery and other community partners to raise awareness about inclusion and accessibility for individuals with Intellectual or Developmental Disabilities/Delays (I/DD).

Case Management Redesign (CMRD) consists of several initiatives that will help make accessing long-term services and supports easier by having Case Management Agencies (CMAs) serve all Home and Community-Based Waivers (HCBS) so individuals and their families can easily access the services they need through a single local agency.
