Family Support Services Program
Our Mission:
The Family Support program provides individualized supports and services to families who are caring for a family member with
developmental disabilities/delays. The Family Support program links families with community resources, and can provide limited financial resources which are beyond
typical child rearing and daily living expenses.
What We Do:
Pathways Family Support Coordinators provide ongoing supportive personal contact with families and facilitate connections with other families and other community
agencies and resources. In partnership with families, Coordinators assist with developing the Family Support Plan by assessing and prioritizing the childs’ and
families’ unmet needs in areas such as assistive technology and home or vehicle modification, respite care, and professional, medical or dental services. Coordinators
also assist the family with planning during times of transition to other school programs and to prepare for additional services individuals may be eligible for in the

Education Committee - In cooperation with our Training Department, the Education Committee is committed to providing
families with training in "quality of life" areas.
Parent Connection - The Parent Connection was designed to give families the chance to connect to each other in a safe
environment, free from stress. Some of the events offered last year include: a Holiday Party*, a Family Picnic*, the Bi-monthly Parents' Night Out and the Monthly
Exceptional Kids Club (*The starred events were also offered in a Spanish-speaking format). These events offer the chance for socialization as well as community inolvement.

Hola! Bienvenidos al departamento de apoyo a la familia! Cordinadoras de apoyo a la familia dan contacta personal a las familias y dan conneciones a otras familias y
otros recuerso y agancias en la comunidad. Junto con familias, Cordinadoras assisten con la developmentacion del Plan de apoyo a la familia por assessando la
necesidades del nino y la familia en areas como technoicia y modificaciones de casa o de transportacion, carino de respite, y servicios de profesionales, medicales,
o dentales. Tambien, Cordinadoras assiten la familia durante tiempos de trancicion a otras programas a la escuela y en preperacion para servicios despues de la edad
diciocho.
El primer paso para familias que tienen hijos con retrasos developmentales o dishabilidads es a contactar Pathways y complitir un aplicacion para servicios yapoyo.
Programas son diference por el edad del nino.
