Enrollment Information for Parents of Children on the Children’s Extensive Support (CES) Waitlist:
- Please note this information is intended as a guide to assist parents in understanding the enrollment process. Please contact your Community Centered Board (CCB) for specific information regarding your situation.
- This is a guide for parents whose child is
currently on the CES waitlist. If you are interested in getting your child on the waitlist, please call your CCB.
The child must continue to meet the targeting criteria for the CES program to be eligible for enrollment.
If it has been more than 6 months since you originally applied for CES, a new application will be required. Eligibility for CES includes:
The child demonstrates a behavior or medical condition that requires direct human intervention on average of every 2 hours during the day. Human intervention must also occur on a weekly average of once every three hours at night. The behavior or medical condition must be considered beyond what is typically age-appropriate.
The child must also continue to meet the criteria for eligibility for a developmental disability.
A CCB representative will check the child’s eligibility for receiving developmental disability services. Redetermination of developmental disability may be needed before the child can enroll in CES services.

Preparation For Enrollment:
There are several things that you can do to prepare for CES enrollment. These steps can expedite the process and will be extremely helpful when you meet with your CCB coordinator.

Documents To Have Before Your Meeting:

- Completed CES Enrollment Checklist
(see a copy of the CES Enrollment Checklist)
- Two forms of supporting documentation of child’s behavior/medical condition
- Copies of any current statements for checking/savings accounts or other finances
(in child’s name only)
- Copy of the SSI denial letter
- Copy of child’s health insurance card
- Copy of child’s birth certificate
- Copy of child’s Social Security card
- Copy of child’s current Medicaid card (if applicable)
- Copy of most recent school IEP
Application/ 24 Hour Log:
The application consists of a 24 hour log. Please fill out the log documenting medical interventions and/or behaviors/vocalizations that would typically occur over a 24-hour period. Please be as specific as you can in describing the behaviors or interventions. Also include what the human intervention was. Include information on Emergency Room visits, hospitalizations, police interventions, unexpected doctor visits, etc. that have occurred over the past 6 months.
Two Forms of Supporting Documentation:
Two forms of documentation are required for the application. Examples of acceptable evidence can include: medical records, professional evaluations and assessments, progress notes, letters/narratives from doctors, teachers or therapists, educational records, communication log between parent and school, insurance claims, Behavior Pharmacology Clinic reports, incident reports, police reports, social services reports or observation by a third party on a regular basis.
Acceptable evidence shall show that behaviors or medical conditions associated with eligibility have occurred within the past six (6) months.
SSI Denial Letter:
Parent(s) should call Social Security at 1-800-772-1213 or apply online at
www.ssa.gov. A letter should be obtained that denies the child SSI due to parent’s income.

Initial Meeting:
The initial meeting with your CCB representative will consist of:
- Completing Medicaid application
- Completing CES application and ULTC 100.2
- Collecting documents from family
Medicaid application:
A CCB coordinator will help you fill out the Medicaid application. Parents should gather any documents, statements etc. for checking accounts, savings accounts, bonds, or any other financial information
that are in the child’s name only. The parents’ income and finances will not be considered for the Medicaid application. Typically, the child will not have any finances; however, there are circumstances where the child may have money in an account. The child cannot have over $2000 to his or her name to qualify for Medicaid.

Eligibility:
After this meeting, the CCB submits the CES application to a peer review organization that determines eligibility. The CCB also submits the Medicaid application to the appropriate county.
A Notice of Service Status letter will be sent to you and your CCB regarding CES eligibility.

Second Meeting:
If your child is determined CES eligible, a second meeting will be held with your CCB representative. This meeting will consist of:
- Developing an Individualized Plan (IP) for your child
- Determining CES funded services. Please see list of
CES Waiver Services for a list of allowable services.
*If the Notice of Service Status letter states that your child is not CES eligible, you may appeal that decision. Directions for the appeal process are included in the letter.
